  • Employee preparation form for probation review meeting

    This form can be shared with employees to enable them to prepare for their probationary period review meeting. The form asks a series of questions to prepare the employee, covering issues such as induction, training and further support.

    Line manager preparation form for probation review meeting
    This form can be used by line managers at a probationary period review meeting to outline progress since the mid-point review, to record whether the employee has successfully completed their probationary period and to outline any areas requiring further development.

    Mid-point probation review form
    This form can be completed during a mid-point probation review to identify any concerns about how the individual is settling in, discuss any issues around performance and to plan any training needs.

    Probationary period review form
    This form should be used to review an employee’s performance during their probation period and recommend whether the employee should be confirmed in their post. The letter also explains that the employee will be given the opportunity to focus on improvement in the areas outlined and that additional support or training will be provided.

    This letter invites an employee to a meeting to review their probationary period. The letter confirms the details of the meeting and also outlines the purpose of the meeting and the possible outcomes of the meeting.

    This policy outlines the process for managing a probationary period, including an explanation of the purpose of probationary periods, how to manage any concerns during the probationary period, what support should be provided, along with information on confirming the successful completion of a probationary period as well as terminating employment either before completion of the probationary period or at the end of the probationary period.
